How do I register with the practice?

Please bring along your medical card which the receptionist can help you complete. If you do not possess a medical card ask at Reception for an application form (GMS1) or you can download one here.

Summary Care Record

Please be aware that every patient registering with the NHS will automatically be opted in to the Summary Care Record Scheme.  This is a Central NHS electronic computer record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had.  Further details on this and how you can opt are found in the 'Summary Care Record' section in 'Further Information'. 

New patients will be asked to complete a personal health questionnaire and to make an appointment with the Practice Nurse for a basic health check. If you have a complicated medical history or take regular medication please make an additional appointment with the doctor after your basic Health Check.

Visitors/Holiday makers are registered on a special form (T/R) provided at Reception.

You will be registering with the Practice and can therefore see any of the doctors in the Practice. However, it is preferable to remain with the same doctor for the duration of treatment of a particular illness and you may select a “preferred doctor” when you register.
